Intake Coordinator

Overview

Are you someone who loves organizing information, coordinating schedules, and helping clients take the first step toward care? As a Care Navigator at Minnesota Mental Health Clinics (MMHC), you’ll play a key role in ensuring clients feel supported and informed from the very first contact. This position blends responsibilities from intake coordination, client engagement, and scheduling management to create a seamless experience for new and returning clients.

Key Responsibilities

Client Scheduling & Intake Coordination

  • Serve as the first point of contact for new clients, responding to intake inquiries by phone, email, or referral.
  • Schedule new clients for brief diagnostic assessments (BDAs), psychiatry evaluations, or trauma-related services.
  • Verify client demographics and insurance, and ensure completion of intake paperwork and required forms.
  • Monitor and manage leads in the CRM system to ensure timely follow-up and reduce drop-offs.
  • Confirm upcoming appointments and send appointment reminders as needed.

Client Engagement & Access Support

  • Deliver a welcoming and professional experience through every client interaction.
  • Proactively identify and address barriers to care, including missing paperwork, expired ROIs, or scheduling gaps.
  • Track and re-engage clients who are missing appointments or falling out of care.
  • Support clients in understanding the next steps in their care journey and who to contact for specific needs.

Collaboration & Communication

  • Communicate relevant client updates to the care team, including reschedules, provider change requests, or follow-up needs.
  • Work closely with intake clinicians, care navigation peers, and practice managers to ensure coordinated support.
  • Assist in optimizing workflows and improving client access across MMHC locations.

Administrative Coordination

  • Track incoming referrals and documentation, including ROI and consent forms.
  • Support scheduling reports and provide updates to management about trends or client needs.
  • Ensure accurate documentation in the EHR and CRM systems in alignment with agency procedures.

Requirements

  • Associate degree required; bachelor’s degree in health, human services, social work, or a related field preferred.
  • 1+ years of experience in client-facing roles, care coordination, or administrative intake support.
  • Strong verbal and written communication skills.
  • Exceptional attention to detail and organizational skills.
  • Comfort with scheduling platforms, CRM tools, and EHR systems.
  • Ability to work independently and as part of a collaborative, high-volume team.
