Job Title: Vice President of Clinical Integration and Innovation

Position Overview:
Minnesota Mental Health Clinics is seeking a strategic and operations-driven Vice President of Clinical Integration and Innovation. This role will lead the design, implementation, of the integrated care function at Minnesota Mental Health Clinics including:

  • Oversight of MMHC’s Care Navigation Team
  • Ongoing care coordination across the internal and external MMHC team
  • Inbound and Outbound client administrative support and coordination
  • Clinical team support and practice management including facilitating internal care coordination meetings and care plan updates

The ideal candidate will be a collaborative leader with a deep understanding of behavioral health systems, clinical operations, and integrated care delivery. This person will play a key role in improving client access, aligning operational workflows with clinical complexity, and ensuring the effectiveness of our evolving care coordination infrastructure.

Key Responsibilities:

  1. Clinical Intake and Internal Referral Operations
  • Design and lead a centralized Care Navigation Team that supports access, client matching, and seamless onboarding into care.
  • Supervise clinical intake supervisors and ensure adherence to best practices in client engagement and intake documentation.
  • Develop and oversee internal referral systems that improve client continuity and optimize utilization of services across MMHC’s offerings.
  • Collaborate cross-functionally with compliance, billing, and technology teams to ensure intake processes are efficient, compliant, and client-centered.
  1. Practice Management and Growth of Care Coordination Services
  • Oversee the development and implementation of a tiered care coordination model aligned with clinical complexity and service mix.
  • Guide the growth of the Care Guide model, ensuring clear role expectations, training systems, and quality standards for care coordination across clinics.
  • Support the development of specialist roles (e.g., housing, benefits navigation, school coordination) that enhance value and scalability of care support.
  • Work with executive leadership to explore commercialization strategies, including care coordination “buy-up” options and B2B service partnerships.
  1. Operational Leadership and System Alignment
  • Partner with IT and operations to build systems that support care tracking, documentation completion, scheduling support, and cross-team collaboration.
  • Utilize data to identify bottlenecks and areas for improvement in care access, coordination, and renewal rates.
  • Monitor and report on KPIs such as renewal rates, client satisfaction (Care Guide NPS), and operational metrics across the intake and care support teams.
  • Ensure all systems align with payer requirements, documentation timelines, and clinical expectations.
  1. Strategic Collaboration
  • Collaborate closely with the VP of Clinical Services and Clinic Directors to align access and care support functions with clinical staffing, program development, and client needs.
  • Partner with billing and finance to explore care coordination billing codes and reimbursement models.
  • Support a culture of cross-functional collaboration, process improvement, and high-quality client service.

 

First-Year Success Metrics:

  • Launch and operationalize a centralized intake system with measurable improvements in timeliness, conversion rates, and client satisfaction.
  • Implement and scale care coordination tiers and specialist roles with defined performance measures and documented impact on client outcomes.
  • Improve internal referral completion rates and reduce service delays or access gaps.
  • Establish reporting infrastructure for key metrics including Care Guide NPS, renewal rates, and scheduling success.

 

Qualifications:

  • Master’s or Doctoral degree in a clinical or healthcare-related field (e.g., Social Work, Psychology, Public Health, Healthcare Administration).
  • Licensed mental health professional in Minnesota preferred.
  • 5+ years of progressive leadership in healthcare or behavioral health operations.
  • Proven experience in systems design, intake management, and care coordination models.
  • Strong collaboration, communication, and project management skills.
  • Experience managing multi-disciplinary teams and leading large-scale process improvements.
