Community Support Coordinator

Minnesota Mental Health Clinics (MMHC) has provided quality outpatient mental health services to clients in the Minneapolis and St. Paul metro area for over 35 years. Job Description

Minnesota Mental Health Clinics is seeking a full time employee for the Horizons Community Support Program (CSP). Horizons is a community support program providing a range of services to seriously and persistently mentally ill adults 18 and older in Dakota County. We have been serving clients in the Twin Cities Metro for over 35 years. We currently have employment opportunities at our Eagan location.


Qualified candidates will possess a BA/BS, must have a valid Minnesota driver’s license and good driving history, and availability to work weekdays, one Saturday per month, and one weekday evening (Wednesday) per month. This position provides services in the Community Support Program such as coaching and redirecting clients in a social setting, running support groups, and providing outreach to clients in the community. Strong candidates also possess effective engagement skills, strong accountability and teamwork.
