Mental Health Intake & Navigation Clinician (Pre-Licensed)

We are seeking a compassionate, motivated pre-licensed Mental Health Intake & Navigation Clinician to serve as the first point of clinical contact for new clients. In this highly impactful role, you will help connect individuals with the right care by conducting clinical intakes, assessing presenting concerns, identifying psychosocial needs, and facilitating referrals to appropriate internal or external services.

Working under the supervision of a fully licensed clinician, you’ll conduct diagnostic assessments, provide initial treatment recommendations, and ensure smooth, coordinated handoffs that support strong engagement in care. Role responsibilities and duties include:

  • Rapidly build trust and therapeutic rapport with clients during initial contact—often during times of high stress or uncertainty
  • Conduct biopsychosocial assessments and mental health screenings under supervision to gather clinical history, assess symptoms, and inform diagnostic impressions in alignment with DHS requirements for a Brief Diagnostic Assessment
  • Evaluate risk, acuity, and psychosocial factors to help guide treatment recommendations and triage decisions, with consultation as needed
  • Support clinically appropriate referrals to internal or external providers based on presenting needs, level of care, cultural fit, and availability
  • Assess client care coordination needs (e.g., housing, transportation, benefits, system navigation) and help connect them to the appropriate level of internal care coordination or external supports to ensure continuity and wraparound care
  • Collaborate with internal teams (case management, peer support, psychiatry, etc.) and external systems (county agencies, primary care, crisis response) as appropriate
  • Provide clear, timely, and organized documentation that meets supervision guidelines and internal policy requirements
  • Participate in case consultation, interdisciplinary team meetings, and ongoing clinical supervision
  • Contribute to ongoing process improvements within intake and triage workflows, sharing feedback and observations from front-end client experiences

Requirements:

  • Master’s degree in Social Work, Counseling, Psychology, or a related mental health field
  • Active pre-licensed status in Minnesota (e.g., LGSW, LPC, LAMFT) and eligible for clinical supervision
  • Basic clinical assessment and documentation skills with a strong desire to grow in diagnostic work and care planning
  • Understanding of—or interest in learning about—community mental health systems and local resources
  • Strong interpersonal, communication, and organizational skills
  • Commitment to equitable, culturally responsive, and trauma-informed care
  • Comfort using Microsoft Office and EHR platforms; willingness to adapt to new systems and workflows
