Horizons provides services and essential support to community members experiencing mental health difficulties β€” helping them live productive and satisfying lives. We pair targeted case management with a Dakota County community support program that builds connection, skills, and confidence.

Horizons Targeted Case Management

A long-term, advocacy-driven relationship for adults with SPMI

We provide contracted Case Management services for both Dakota County and Hennepin County Social Services. Case Managers provide information, referrals, and advocacy in the context of a supportive long-term relationship β€” and help members set realistic goals and work towards them in a planful way.

  • HousingAssistance finding adequate housing, applying for housing subsidies, and in-home support to maintain community living.
  • EmploymentReferrals and support across obtaining and maintaining competitive employment and volunteer positions.
  • Benefits AssistanceHelp obtaining subsidies, financial aid, and entitlements.
  • Case ManagementContracted case management for Dakota County and Hennepin County Community Services.

Who we support

  • Dakota & Hennepin County residents, age 18+
  • SPMI diagnosis: Schizoaffective Disorder, Schizophrenia, Major Depressive Disorder, Bipolar Disorder, or Borderline Personality Disorder
  • Medical Assistance (MA), or qualifying for an MA exception

Horizons Community Support Program

A welcoming place to build skills, confidence, and connection

Located at our Eagan office, the Community Support Program helps adults living with mental health challenges build the skills, confidence, and support they need to live well within the community. The Horizons Drop-In Center is available for Dakota County residents.

Drop-In hoursMonday – Thursday, 10am – 4pm

Members gather to play pool and cards, do arts and crafts, attend support groups, socialize, and plan offsite activities. All Horizons services are available on site during drop-in hours. We serve adults 18+ with Severe and Persistent Mental Illness (SPMI) β€” everyone welcome.

  • EducationLinks members with community educational resources and offers informative groups that members participate in and help facilitate.
  • Social & RecreationalA wide range of community events and drop-in activities chosen and planned by members.
  • OutreachSupport and assistance that reduces social isolation for members.
  • Friendship ProgramConnects volunteers with isolated members of the community who are experiencing mental illness.
  • TransportationTransportation for members in southern Dakota County to Horizons and to Horizons-organized community activities.
